1. Who we are
This website is operated by Northline Referral Partners ([Legal Entity Name]) (“Northline Referral Partners,” “we,” “us”). You can reach us through the contact form.
2. Who this site is for
This site and our services are for medical practices and their business representatives. We don’t provide medical care and don’t offer services to patients.
3. Information we collect
- Business contact information you choose to send us through the contact form: name and role, organization, business email, business phone, specialty, city/state/region, objective, preferred contact method, an optional message, and your consent confirmation.
- Basic technical information your browser sends to any website, such as IP address and browser type, which our hosting provider may log for security and operations.
- Spam-prevention signals (a hidden field and the time taken to submit the form), used only to filter automated submissions.
4. Patient information
Please don’t submit patient information or any protected health information (PHI) through this site. Our forms aren’t designed to receive PHI, and we don’t accept patient referrals here. If PHI is sent to us by mistake, we will delete it and won’t use it. For background on HIPAA marketing rules, see the HHS guidance linked below.
5. How we use information
- To reply to inquiries and schedule strategy calls.
- To provide our services to practices.
- To meet our legal obligations.
We don’t sell personal information, and we don’t use it for advertising.
6. Sharing
We share information only with service providers that help us run this site and our business, such as hosting, form processing and email, and only as needed for them to do that work under confidentiality obligations. We may also disclose information when the law requires it.
7. Cookies and analytics
This site doesn’t set cookies and doesn’t use analytics or advertising trackers.
8. Retention and security
We keep inquiry information only as long as we need it to respond and to maintain a business relationship, and we use reasonable administrative and technical safeguards to protect it.
9. Your choices
You can ask us to access, correct or delete the business contact information you’ve sent us by writing to us through the contact form.
10. Changes
If we update this policy, we’ll post the new version here with a new “last updated” date.
